Dear Franklin Families,
The Partnership for Public Health has scheduled an influenza vaccine clinic for students on the following dates:
Franklin High School - October 17, 2023 @ 8-11:30a.m.
Paul Smith School - October 18, 2023 @ 8-11:30a.m.
Franklin Middle School- October 19, 2023 @ 8-11:30a.m.
If you would like your student to receive an influenza vaccine, consent forms and information can be accessed using these links:

NHIIS Info for Patients and Parents
If you would like your student to receive this vaccine, please print and complete the consent form along with the opt-in or opt-out NH Immunization Information System (NHIIS) form in their entirety and return it to your student’s school nurse.
If you do not consent to this vaccine, there is no need to return the forms.
Packet’s with forms may be picked up in the main office of your child’s school.
A reminder email will be sent to you 10 days before the clinic.
